This Group Learning program has been certified by the College of Family Physicians of Canada and the Ontario Chapter for up to 5Mainpro+ credits.

Jordan Zaitzow – MSW, Training Coordinator for Trans Health Connection, experienced workshop facilitator and trans health educator

We will be sharing information on:

• Providing holistic, community-based primary care services to trans patients
• Initiating hormone therapy for trans patients, including specifics on pre-hormone supports, medication doses, and monitoring
• Disease prevention / screening guidelines for trans and gender-diverse patients
• Reviewing new provincial trans health protocols: shifting the lens on trans care, and integration into “sensible practice” and informed consent
• Available community resources and when / who to refer to for help in managing trans patients’ care
• Building a locally-relevant, integrated health system for gender diverse communities
